MN LTSS Community Partner Survey

The Minnesota Legislature established a Long-Term Services and Supports (LTSS) Advisory Council (Council) to identify strategies to reduce cost growth in LTSS and produce viable strategies for consideration by the Legislature.  The Council includes representatives from individuals who receive LTSS, family caregivers, providers, and unions. As they identify and finalize recommendations for the Legislature, the Council values:
  • The well-being of participants and workers,
  • The stability and financial sustainability of Minnesota’s LTSS,
  • Participant access to services and choices, and
  • Participant quality of life.
Reason for this Survey: This survey will gather information from partners, families, and individuals who are impacted by LTSS. The feedback provided will help the LTSS Advisory Council shape their final cost savings recommendations.

Cost savings may be found by adjusting eligibility standards, reducing or revising the types or amounts of services offered, and/or adjusting provider rates. YOUR feedback will be reviewed by the LTSS Advisory Council before they finalize their recommendations.

You can switch the survey to Hmong, Russian, Somali, or Spanish by selecting the language menu in the upper right corner, where the globe icon appears next to “English.”
You can also turn on Low Vision Mode using the eye icon in the same corner. This mode increases contrast, font size, and font weight to make the survey easier to see.

All responses provided are anonymous and you may skip any question you do not wish to answer. We appreciate any feedback you are willing to provide.
