Welcome to the City of Phoenix Landlord and Tenant Education Request Form.

This form is intended for tenants, landlords, or representatives seeking education about landlord and tenant rights and responsibilities under the Arizona Residential Landlord and Tenant Act (ARLTA).

Please complete this form with accurate contact information. A City of Phoenix staff member will follow up within 48 business hours to provide educational resources or referrals.

Tenants – If you are experiencing a maintenance emergency or urgent housing issue, please complete our Emergency Assistance Intake Form instead to ensure timely support via LTP Emergency Assistance Request Form. Maintenance emergency or urgent housing issue includes:
  • No air conditioning or heat
  • Plumbing or electrical problems
  • Roof leaks or water damage 
  • Other unsafe living conditions
