AMP Mentee Application

Welcome

Thank you for your interest in the Autism Mentorship Program (AMP). Please visit our Eligibility page to review the eligibility criteria for mentees.

We’re excited that you are interested in being mentored! We would like to know more about you. This is the first step in the 2-part application process: 1) this online application and 2) a virtual interview to meet you and discuss this application.

This application should take approximately 20 minutes to complete. The APPLICANT (young person applying to be a mentee) and their GUARDIAN (unless the applicant is 18-22 and their own guardian) will be asked questions on this application. We will use this information as a starting point for your interview to learn more about you and what qualities would be a good fit for a potential mentor.
