Marion Public Health is conducting a survey as part of the upcoming Community Health Assessment. We are seeking input from seniors about their health, well-being, daily habits, and experiences. Your feedback will help identify community strengths, needs, and priorities, and guide efforts to create a healthier, more equitable Marion for all.
All survey respondents can be entered into a $50 gift card drawing if they choose to do so. You must be at least 18 years or older to be entered to win (if selected as a winner, age verification is required.) In order to be entered into the drawing for a $50 gift card, please complete all questions to the best of your ability and provide your name and phone number on the last page. Your personal information will not be included in the analysis of survey results.
