Welcome to the 2026 Children’s Hospital Colorado Community Health Survey.
This short survey will help us better understand and guide efforts to address health challenges for children and youth in your community. To be eligible to complete this survey, you must be over 18 and have children or youth in the household or regularly in your care. The survey should take less than 10 minutes and should be completed in one session. The information collected will help us:
• Understand challenges that affect our community,
• Better understand the needs of our community, and
• Work together to find solutions to our needs.Your voice is important and will help us better understand the health and social needs of our community. We recognize that some questions may be personal. Responses are completely anonymous and cannot be linked back to you. We encourage you to answer each question honestly and based on your own experiences, as every perspective matters.
