Buy American Provision Waiver Request Form

Please complete this form to request a temporary accommodation under the Buy American Requirement for your school food authority (SFA) for SY 2026-27. Submissions will be reviewed by the DPI Bureau of School Nutrition, and you will be notified of the decision within 10 business days. If approved, technical assistance will be provided to support your transition toward compliance.
The following information is required to complete this form:
  • Agency Name
  • Agency Code
  • The categories of food that require an accommodation from the threshold requirement (i.e., exceeding 10 percent)
  • The reason(s) for requesting an accommodation.
  • The anticipated timeline for making modifications to meet the Buy American requirements.
  • (Optional) A PDF of documentation you have to support your request
