Wisconsin Statewide EHR Interest Survey

Statewide EHR Interest Survey

As part of the Rural Health Transformation Program (RHTP), the Wisconsin Department of Health Services (DHS) is creating a statewide electronic health record system (EHR) to facilitate high-quality care for patients in rural and semi-rural Wisconsin through new infrastructure that allows for better and faster interaction between providers and patients.

Eligible organizations must be in rural or semi-rural counties and perform ambulatory services. Organizations that provide inpatient services are not eligible under this program.

If you have interest in participating, please share a few details about your organization to help us understand your readiness and interest. Completing the survey does not commit your organization to participate.

This program is supported by the Centers for Medicare & Medicaid Services (CMS) of the U.S. Department of Health and Human Services (HHS) as part of a financial assistance award totaling $203,670,005.21 with 100 percent funded by CMS/HHS.
