The South Carolina Statewide Independent Living Council (SCSILC) wants to better understand the needs of people with disabilities across the state. This survey asks about your experience as an individual with a disability, any barriers you may face, along with any services you have received.
Your honest feedback will help SCSILC understand what is working well and what needs to improve to better support people with disabilities in South Carolina.
Please only take this survey if you are a person with a disability or taking the survey on behalf of a person with a disability.
Some information to know before you begin:
- All responses are private and staff do not have any way of knowing who you are. This allows you to provide honest answers to all the questions.
- The survey should not take more than 15 minutes to complete.
- The survey will be open until October 15, 2026.
- Spanish or printed versions of the survey are available.
Completing this survey is your choice, though sharing your insight is incredibly important.
