This form is intended for non-state contracted community-based organizations requesting participation from a CDSS CalFresh Outreach contractor at an upcoming community event. Outreach support may include CalFresh outreach, application assistance, educational presentations, or participation in resource fairs and other community events.
Please complete this form with as many details as possible to help us identify the appropriate outreach contractor and assess availability. Submission of this form DOES NOT guarantee participation.
