Chaffee County Public Health is preparing for its next 5-year Community Health Improvement Plan. Your input into the top health concerns affecting you, your neighbors, family, and friends is critical to shaping our work priorities moving forward.
The last assessment and improvement plan process from 2022-2026, for example, determined the top health needs for our county as 1. Access to Healthcare, 2. Behavioral Health, and 3. Environmental Health Stewardship. We made noteworthy progress maximizing resources and support, and collaborating with key partners, organizations, and community members.
A lot has changed in the last five years, post-COVID, and we at Chaffee County Public Health want to be as prepared, responsive, and equitable in our programming, services, and outreach as possible. Your experiences are so important to our perspective and understanding. Please take this survey and share the link with others in your community.
Survey Details:
- Anonymous & Voluntary: Your responses are confidential and combined with others for analysis. This survey is intended for residents of Chaffee County who are 18 years of age and older.
- Timeframe: Open from July 27, 2026- to September 30, 2026 and takes about 6-8 minutes to complete.
- Incentive: Enter to win a $10 gift card by sharing your name and email after completing the survey (optional). The winner will be contacted within two weeks of the survey closing.
