Keep Your School’s Information Up to Date

Private School information is updated manually twice each month in both the State Board of Education files and OSPI's EDS System. Accurate school contact information helps ensure that your school receives important updates such as Private School Bulletins, compliance reminders, and other critical communications.

Schools can double check the information the State Board of Education has on file by either referring to their Approval Application OR by checking the Approval Application Status Tracker on the Private School Approval | SBE webpage.

Before submitting, please double-check all entries, especially email addresses, to avoid processing delays.

School address
Main office phone number or email
Head of School

No changes? No form needed.

If your school’s leadership or address has not changed since submitting the 2026–27 Private School Approval Application, you do not need to submit this form.
