FY2026 CoC NOFO: Local Competition New and Transition Project Application Submission

Instructions:
  • Please complete all required questions within this survey to be considered for New or Transition project funding through the FY2026 CoC funding process of the Opening Doors of Fairfield County (ODFC)/CT-503 Continuum of Care (CoC).  
  • NOTE: Only questions that apply to your selected project type will be displayed.
  • All applications must be submitted via Alchemer no later than 5:00pm EST on Monday, July 27, 2026.
  • Please carefully read the CoC’s FY26 CoC Competition New and Transition Project RFP before completing this survey. The RFP can be found here.
  • You may click “Save and Continue” in the upper right-hand corner at any time to save your responses and return to complete at a later time- a link will be emailed to you to return to your responses. If you do not receive an email with link, email OpeningDoorsofFairfieldCounty@gmail.com. HOWEVER, we highly recommend that you work on a hard copy of your responses first prior to entering your responses in Alchemer -- in case of a technical issue we do not want you to lose your work. A link to a hard copy of the application can be found below and in the RFA.
  • Upon submission, a copy of your responses will be emailed to you for your records.

GENERAL INFO:
  • ONLY ONE NEW PROJECT APPLICATION CAN BE SUBMITTED IN EACH SURVEY. If you plan to submit multiple new project applications, you will need to complete a survey for each separate New Project Application. 
  • Existing CT-503 CoC grantees with Rapid Rehousing (RRH) projects are encouraged to submit New Project Applications using the Transition, DV Reallocation, or YHDP Reallocation processes, as appropriate, to convert to a Transitional Housing model. 
  • If you are an existing CT-503 CoC grantee with a PSH project and your organization would prefer to keep your existing PSH renewal but you would transition your project to a TH model if necessary, you should submit both a Renewal Project Application and a New Project Transition Application. 
  • All New Project Applicants are encouraged to carefully review the FY26 New Project Scoring Tool, which will be used to evaluate and score all New Project Applications.
