AHS & SAGES Hernia & Abdominal Wall Fellowship Certificate Application

Application Deadline: September 30, 2026

The American Hernia Society (AHS) and the Society of American Gastrointestinal and Endoscopic Surgeons (SAGES) award the Hernia & Abdominal Wall Fellowship Certificate to graduates of Fellowship Council-accredited programs who have successfully completed the established requirements for advanced training in hernia and abdominal wall surgery.

Please complete the application below to verify your successful completion of fellowship training and document that you have met the established certification requirements. Applications will be reviewed by the AHS & SAGES Certificate Committee, and eligible applicants will be awarded a Hernia & Abdominal Wall Fellowship Certificate recognizing their knowledge, technical expertise, and professional competency for independent practice.

A copy of your application will be sent to your Fellowship Program Director to verify the accuracy of your submission and attest that you have successfully completed the fellowship requirements. Final certificate approval is contingent upon this verification.

Operative Experience
Please enter the total number of procedures completed in each category. To be completed as Primary Surgeon or as Teaching Assistant. If cases are performed robotically, the fellow must be the console surgeon for >50% of the case.

Please Note:

  • Inguinal Hernia Repairs (min 30)
    • 15 of these must be done via an MIS approach (Laparoscopic or Robotic)
    • 10 of these must be recurrent hernias
  •  Ventral/Incisional Hernia Repairs (3-10 cm width or length) (min 30)
    • 15 of these must be performed as MIS extraperitoneal
