2026-2027 Annual K-12 Health Assessment and Immunization Report

Welcome! Before you proceed to the online Annual K-12 Health Assessment and Immunization Report, please read the instructions carefully. 

You will have the option to download a copy of the health assessment and immunization worksheets that can be helpful to gather your data before entering your data into this online report. Do not mail or submit the physical worksheets; keep them for your records. Public schools must complete the Health Assessment, and will be automatically directed to the Immunization Report afterwards; all schools must complete the Immunization Report. After you enter your school information, you will be directed to the applicable reports automatically. ​​​​​

Your report will not be complete until you complete all required questions, and click the "Submit" button on the last page. Ensure you "Submit" for compliance purposes.

Please submit a report even if no children are attending your school or your school is closed for the 26-27 school year; you can indicate that your school is closed or has zero children when prompted in the online report. This will ensure your center meets the reporting requirement and is not considered delinquent.
