Transforming Maternal Health (TMaH) Survey

Transforming Maternal Health Survey

The Transforming Maternal Health (TMaH) model seeks to deliver whole-person care, increase the quality of maternal health care, and improve access to maternal health services and personnel across Wisconsin. Wisconsin is one of fifteen states to receive TMaH funding from the Centers for Medicare & Medicaid Services (CMS). Your survey responses will be used to complete maternal health landscape assessments to prepare for TMaH implementation.

Thank you for taking the time to complete the Transforming Maternal Health survey. The survey includes questions about the maternal health workforce, quality improvement, remote patient monitoring, and screening and referral processes for behavioral health and health related social needs. If you are not familiar with a topic, you can skip that section of the survey. Please note that your responses to this survey will be confidential unless required by law. Do not include patient PHI/PII in your responses. The data will be analyzed for trends and summarized with a publicly available report.

We are asking providers who work with pregnant and/or postpartum people as well as quality improvement staff and administrators who work in maternal health to respond to this survey. For most providers, this survey will take approximately 20 minutes to complete. Please only respond to the survey once.
