OAS Access Quiz

This quiz is required for both Choice administrators and designees. 
  
Required Forms 
  
In order to receive an OAS user name and password, the school must submit one of two forms according to the person's role in the Choice programs.  Each of these can be found on our School Submitted Reports and Forms webpage.   

Choice Administrator: Choice Administrator Authorization and Update Form 
Designee: Designee Authorization and Update Form 

Regardless of the form used, the person completing the form must attest to the fact that he/she has EITHER read the required trainings and successfully completed the quiz or quizzes OR has been a Choice administrator or designee (as applicable) within the past two years.

Required Trainings and Quizzes   

Choice Administrator: Complete both the OAS Access Quiz and the Program Requirements Quiz.  The required trainings and quizzes can be accessed through the Choice Administrator Training and OAS Access web page.   

Designee: Only required to complete this quiz, the OAS Access Quiz.  The required trainings and quiz can be accessed though the Designee Training and OAS Access web page.    

The quiz asks questions about topics covered in the trainings.  At the end of the quiz, you will have the opportunity to download and print or save the correct responses. 
  
If you choose the wrong answer to a question, you will be asked to try again.  Simply re-read the question and select the correct answer.  You will not be able to advance to the next question until you have correctly answered the current question.
