Welcome to the 2026 Northwestern Medicine Community Health Needs Assessment Survey.
This short survey (less than 10 minutes) will help
guide efforts to address health challenges and create meaningful solutions for the needs of your community. The survey should be completed in one session. The information will help us:
- Understand challenges that affect our community
- Better understand the needs of our community
- Work together to find solutions to our needs
The survey is voluntary, and you do not have to take part. You can also skip any questions you do not want to answer or stop the survey at any time.
The answers you give are very important to us. Your answers will be private. We will not collect your personal information and we will not share how you answered the survey with anyone.
