Cornerstone 2026 End-of-Summer Supervisor Report

Thank you for taking the time to complete this form. Your feedback is important to us as we continue to strengthen the Cornerstone Fellowship program and to support your camp in the future.

Completing your Report

Note that this form should be completed by your camp’s Cornerstone Supervisor. There is a separate form to be completed by your camp’s Lead. If you acted in both roles, please fill out this form, and have your boss fill out the Lead report.
Please respond to all questions in as much detail as possible. For some questions, we've provided tips below answer boxes to provide clarity.

Both End-of-Summer forms (one for Supervisor and one for Lead) must be submitted before
Wednesday, September 30, at 11:59pm PT.
