Quarterly Reporting Instructions – FY27 School-Based Prevention Services Grants
Each quarter, you’ll be asked to report the following:
1. Participant Counts by Strategy
Report the number of participants for each prevention strategy implemented during the quarter.
2. Unduplicated Student Participation
Report the number of unduplicated students who participated in any prevention activity during the quarter.
3. Document Uploads
You may upload up to 5 supporting documents each quarter.
Site Visits
During site visits, we’ll discuss your prevention efforts in more detail, including successes, challenges, and implementation strategies.
