Letter of Intent for CHW Grants

Thank you for your interest in the Community Health Workers (CHW) Grants: Strengthening the rural health workforce in Wisconsin.
 
This survey is intended to collect letters of intent from organizations interested in participating in the CHW grant funding opportunity.
We recommend but do not require a letter of intent to submit an application in response to this grant funding. Letters of intent allow The Department of Health Services (DHS) to better understand the geographic distribution of interest across Wisconsin’s rural areas and helps DHS prepare for application reviews and plan for grantee training and technical assistance. The letter of intent should be provided through this form by July 1, 2026.

Please note: Submission of a letter of intent is not an application for funding and does not guarantee eligibility, selection, or award of funds. To apply for funding, organizations must complete the CHW Grants: Strengthening the Rural Health Workforce in Wisconsin Application.
