Assumption of Risk
I, the undersigned parent/legal guardian, understand that participation in the Treviso Bay All-Sports Camp may involve physical activities including tennis, pickleball, bocce, recreational games, fitness activities, and other sports-related programs. I acknowledge that participation carries inherent risks, including but not limited to falls, collisions, equipment-related injuries, illness, and other unforeseen accidents. I voluntarily allow my child to participate and assume all risks associated with these activities.
Release of Liability
To the fullest extent permitted by law, I, the parent/legal guardian, release and hold harmless Treviso Bay Country Club, its employees, instructors, volunteers, agents, and representatives from any claims, demands, or causes of action arising from my child's participation in the camp, except in cases where liability cannot legally be waived.
Medical Authorization
In the event of an emergency, I, the parent/legal guardian, authorize camp staff to obtain medical treatment for my child if I cannot be reached immediately. I understand that I am responsible for any medical expenses incurred.
