PICPA 39th Annual Volleyball Tournament - Team Information Form

Please complete this form to share your team name, team captain, and player information by the end of the day on Thursday, July 23. To ensure your team is fully registered, each player must also purchase their individual registration through the PICPA.org website. The team guidelines are as follows:

Team maximum: 6 players per side (you can play with fewer).
One female player must be on the court at all times.
Firms are encouraged to enter more than one team, or combine with other firms in order to make a team. To make a complete “firm team,” non-CPA employees are welcome.
Playoff format will be determined by number of teams entering (round robin, elimination, etc.)
