The purpose of this form is to better understand your district's use of School-Based Mental Health Services State Funding during this school year. While the form does ask for a primary contact, we encourage this to be collaborative to fully capture the work that has been done in your district.
You will be asked about your district's size category, so you will need to know your 2025-26 enrollment - which can be searched for on
WISEdash (select "filter data", then "district" and select your district).
It may also be helpful to view or print out
a copy of the form to make responding within the survey easier. We ask that the survey be completed by
August 1, 2026.
