2026 Annual Report for Midwifery Practice

2026 Annual Report of Midwifery Practice Digital Form

Per Rule 64B24-7.014, Florida Administrative Code, each licensed midwife, temporary certificate holding midwife, and midwife supervising a student midwife in assisting in childbirth that occurs in an out-of-hospital setting, must file an annual report no later than July 31 for the prior fiscal year (July 1 through June 30). The data is used to help the Council of Licensed Midwifery observe general trends within the profession and assess regulatory response. Failure to submit this Annual Report of Midwifery Practice will result in a notice of noncompliance issued by the Department.
