This nomination form is supported by Alchemer survey software. Once you begin the submission process, you must complete it for your nomination to be submitted and accepted. If you exit the form before completing your submission, your information will be lost, and you will need to start over.
Before you begin, please ensure you have the following information ready:
- Name, title, affiliation, email, and phone number of your candidate.
- Your name, title, affiliation, email, and phone number (you will be considered the primary nominator.
- (Optional) Names, titles, affiliations, and emails of any additional nominators who are jointly endorsing this candidate.
- A summary of 400 words or fewer describing the nominee’s relevant contributions and accomplishments, and their direct impact.
- A summary of 400 words or fewer describing the nominee’s record of service to the profession.
- A summary of 400 words or fewer outlining the scope of the nominee’s achievements and their enduring influence.
- (Optional) A link to, or PDF file (no larger than 2,000 KB) of the candidate’s résumé, CV, or biographical summary.
- The TRB Award/s you believe your candidate is eligible to receive.
At the conclusion of the nomination process, you will be asked to affirm your endorsement of the candidate’s character.
