Student PE Standards Committee Application

Directions for submitting the application:

Review the Physical Education Standards Committee Guiding Assumptions for Students.
Submit the completed student application if you will be a high school student in October 2026 or contact mde.academic-standards@state.mn.us
The application packet must be received by midnight on August 2, 2026.
The application packet must include one copy of each of the following:
•    Completed application 
•    Academic reference
•    Electronic signature of the student, parent/guardian, and principal

All electronic filenames must include applicant’s full first and last name.

If you are not a high school student, complete the adult application.

In accordance with the Minnesota Government Data Practices Act, MDE is providing you with notice via this Tennessen Warning of: 

The purpose and intended use: The purpose of this form is to collect the information needed to determine the Physical Education Standards Committee. The information will be reviewed by Minnesota Department of Education.  The information you provide will be retained as part of MDE's applicant records for the period required by the State of Minnesota records retention schedule. 

Note regarding private data on minors: You have the right to request that parental access to your private data be denied. If you wish for MDE to withhold your private data from your parent, you must submit your request in writing and include your reasons for denying parental access to your private data. You must also sign your written request.

Whether providing this information is voluntary or required: Providing the information on this form is voluntary. You are not legally required to submit this form. 

Consequences: If you choose not to provide the information requested, MDE will not appoint you to the committee. There are no other consequences for declining to submit this form in full.

The identity of other persons or entities known to be authorized by law to have access to the data: The information you provide may be accessed by:

  • Academic Standards and other MDE staff with a business need to review access provisioning
  • The Office of the Legislative Auditor or State Auditor as part of authorized audits
  • Other parties pursuant to a court order

There are no other individuals or entities authorized by law to have access to this data.
