FY2026 ODFC/CT-503 CoC Renewal Project Evaluation & Scoring Standards which are being used to guide the 2026 project evaluation and scoring process.
Please note that you are not required to respond to every question in this survey. Required questions are marked with a red asterisk. All other questions are optional and text boxes are available so that you can provide any relevant information that your agency believes may impact and should be factored into the evaluation of specific criteria. You may leave the responses to non-required question blank or write "NA" if you have no comments to submit for those questions.
