About This Survey
Population Services International (PSI) and Metrics for Management (M4M) are conducting a study to better understand women's experience of healthcare. We want to ask you some questions about your most recent interaction with a healthcare provider. The survey should take 20-30 minutes to complete. At the end, if you agree, we will collect your name and phone number in order to send you Q25 in airtime as appreciation for your time.

This may have been with a doctor or nurse, pharmacist, community health worker, or any other professional healthcare cadre. It may have been at a health facility, a pharmacy or drug shop, in the community, or even in your home. Was this most recent interaction in the past 7 days, in the past 14 days, or longer than 14 days ago? *This question is required.
4. Where did you receive care: at your home, or did you go to a facility like a hospital, clinic, pharmacy, shop, or other location? *This question is required.
