Introduction
This template is available anytime you need to create a survey for a specific use-case that isn't addressed by existing templates. It has the standard branding and display settings we recommend for all surveys, as well as a few basic suggestions for layout and content.
Use this page to provide a brief description of your survey, including its purpose and goals. Also, be sure to include the deadline for submitting a response, if applicable.
We also recommend including this statement:
"Please note that you may save your progress on this survey by selecting the Save button at the bottom of the screen and providing your email address. Alchemer will send a link to your email that allows you to pick up where you leave off with this survey. Additionally, please note the option to switch to Low Vision mode for this survey."
