This survey form is for reporting changes to the Early College program since the time of initial Early College Designation application review. The form is for major program modifications to be made for the School Year 2026-2027.
The Fall cycle of this form opens on September 14, 2026. T
he last date the form will be accepted for Fall consideration (for changes to be implemented spring 2027) will be October 16, 2026.
The Spring cycle of this form will open on March 10 and be due
April 15 (for changes to be implemented Fall 2027).
The purpose of this form is to ensure that any major changes to the Designated program, whether pertaining to partner or program are aligned with the Guiding Principles and Designation Criteria outlined by the Early College Joint Committee and enhance the program in an intentional way.
Please note, minor/temporary programmatic changes can be noted in the Early College Annual Report.
The form is organized by two types of Major Changes: Partnership Changes and/or Programmatic Changes
Major Partnership Changes may be further broken down to include:
- Adding a new IHE partner with no prior Early College Designation
- Adding a new IHE partner with a prior Early College Designation
- Exiting a Secondary Partner
- Dissolving an Early College Designation
Please note for programs looking to add a new K-12 partner, the program modification process does not apply. These partnerships must complete a commitment form and undergo the Program Design process.
Major Programmatic Changes are organized by Guiding Principles and may be further broken down to include:
- GP.1. Equitable Access: Changes to program recruitment or enrollment plans
- GP.2. Guided Academic Programs of Study: Changes to Academic Programs of Study, including any additional academic pathways or programs of study; measures of college readiness for college coursework; scheduling; coursework modalities, location and instructors; preparatory coursework; additional academic policies
- GP.3. Student Supports: Changes to the structure or major changes to the delivery of academic or non-academic support services
- GP.4. Connections to Careers: Changes to structure of delivery of career awareness, exposure, or immersion activities
- GP.5 Effective Partnerships: Changes impacting MOU, staffing, collective bargaining agreements or evaluation plan.
