2026-2027 Driver Education Program Approval

Information Due Date: August 31, 2026

This Driver Education Approval Application Form is being sent to the District Superintendent (or organizational leader/administrator). The Superintendent/Administrator is responsible for completing this Driver Education Approval Application Form. Assistance from the supervisor of the Driver Education Program may be required.

The application consists of three sections, which must be completed as follows:
  1. Program Assurances
    • All program requirements and standards are outlined in PI 21 and State Statutes 115.28(11) and 121.41.
    • This section highlights the most critical elements for assurance verification.
  2. Participating High Schools and Instructors
    • Public School Districts: List all high schools within the district that will be offering driver education instruction.
    • Private High Schools: Provide the name of the high school.
    • Instructor Information: Include the name, email address, DPI Teacher License File number, and 450 Certification expiration date for all instructors.
    • If instructional services are contracted, specify the employer of the instructor(s).
  3. Driver Education Program Contact Person(s)
    • Provide the name(s) and contact information for the designated program contact person(s).
