Share Your Allergic Fungal Rhinosinusitis (AFRS) Story

Health Stories Project brings together over 60,000 people dedicated to sharing their stories in both paid and volunteer opportunities to inform and inspire others.

1. Have you ever been told by a healthcare provider that you have chronic sinus disease with nasal polyps (growths in the nose or sinuses)?  *This question is required.
Have you ever been told by a healthcare provider that your sinus condition is related to a fungal allergy or fungal sinus infection (sometimes called allergic fungal rhinosinusitis or AFRS)? 
2. If selected for this study, would you authorize us to contact your healthcare provider to verify your diagnosis or treatment, if needed?  *This question is required.
