2025 PA Injured Worker Medical Accessibility Survey

Welcome to the Pennsylvania Injured Worker Medical Accessibility survey! This questionnaire is intended to provide Pennsylvania workers who were injured on the job and received medical treatment the opportunity to share their input to help improve the quality and availability of healthcare for work-related injuries. Your answers to the following questions will help us understand what matters most to you and your family throughout this challenging time.

We ask that you share your honest and candid opinions and encourage you to use the ample blank space provided in the questionnaire to offer additional suggestions. All responses will be carefully reviewed and considered in the reporting of the final results. This survey is required as part of the Workers' Compensation Act and is conducted annually by the independent research firm FieldGoals.US.
