You are invited to participate in our "Child Care for Tradespeople" survey, conducted by researchers at Illinois Action for Children (IAFC), in partnership with Chicago Women in the Trades (CWIT).
We are interested in learning about your experiences with child care as a tradesperson.
CONSENT FORM
This survey should take about 10 minutes from beginning to end.
The first 100 respondents will get a $30 e-gift card as a thank you for their time spent completing the survey. In addition, all respondents will be entered into a raffle for one of ten $100 gift cards. Within two weeks, a gift card will be sent to your email. Please note, responses will be screened for validity and only valid survey responses will receive a gift card.
Participation is completely voluntary, and you can skip any questions you do not want to answer.
Your answers will be kept secure and confidential.
Survey results will be reported in a summary form and any reports will be written in a way that no one can identify you or your answers.
