This evaluation contains questions regarding the RC Program Clinical sites. The feedback from your evaluation is important to us as we continue to improve our program. Your responses will remain anonymous.

Choose one clinical site below that you visited THIS SEMESTER and complete the survey by using the mouse to click on the button that best represents your response. If you choose "Disagree" or "Strongly Disagree" on any statement, please explain your choice in the comment box below the question.
