IMPORTANT NOTE
ONLY Choice administrators may complete and submit this form.
- Choice administrators use this form to report the following:
- Change in school's name or phone number
- Addition or removal of a school location
- Change in grades offered to MPCP, RPCP, and/or WPCP students
- Change in all grades offered by the school (you cannot use this form to change number of seats per grade)
- Change in school term end date
A school cannot change its Federal Employer Identification Number (FEIN) through this form. If a school is considering a FEIN change, contact DPI at
DPI.PrivateSchoolChoice@dpi.wi.gov for information about next steps.
If the school participates in the Special Needs Scholarship Program (SNSP) and the change also applies to SNSP, the school must also complete an
SNSP School Information Update form.
Changes made through this Private School Choice Programs School Information Update form only impact the school’s information for the Private School Choice Programs. Changes made through this form do not change the school’s information on the
DPI School Directory. If the change is also needed for the
DPI School Directory, please see the
School Modification webpage for more information about submitting changes through the DPI School Directory application. If you are unsure if this change is required to the DPI School Directory, the
School Modification webpage describes which changes would also need to be made to the school directory information.
The following changes are made on different forms that are referenced below. These forms are available at
https://dpi.wi.gov/parental-education-options/choice-programs/school-reports.
- Choice administrator change or contact information change can be made on the Choice Administrator Authorization and Update Form
- Choice designee contact change can be made on the Designee Authorization and Update Form
- Governing Board member addition, removal, or contact change can be made on the Governing Board Information Update Form
All questions are required unless marked "Optional."
