Application Form for Governance Committee Position 2026-27

Candidates interested in applying for a position as an ADHA  Governance Committee member must complete the following application form.

The following application offers an opportunity to know more about you, your attributes, skills and experience. 

The Governance Committee oversees the nomination and vetting process, evaluates candidates against identified skill sets, and recommends a slate of qualified individuals aligned with ADHA’s Bylaws and strategic plan. This process ensures the Board maintains a balanced mix of skills, experience, and perspectives to effectively govern the Association.   

Deadline for submission is April 2, 2026.
