The Summer BreakSpot is a federally funded program that provides nutritious meals, at no cost, to children 18 and under while school is out for the summer.
This questionnaire is designed to help the Palm Beach County Food Bank determine if your Agency meets the basic criteria to become a Summer BreakSpot site. You will first be asked to provide information about your organization, followed by details about the site where meals will be provided. If you plan to request meals for more than one site, please complete a separate form for each location.
As a Summer BreakSpot partner, you agree to:
-Attend a Summer BreakSpot training (including all staff participating in the program)
-Supervise activities and meal service at the site
-Manage volunteers/staff
-Distribute meals in accordance with SFSP guidelines and ensure they are consumed on-site
-Keep daily records of meals served
-Store food appropriately
-Keep the site safe, clean and sanitary
-Allow site visits
-Promptly communicate any changes to PBCFB
-Display appropriate posters and signage at the meal distribution location
-Promote the program in the community
