2026-27 DOD Fresh Fruit & Vegetable_Enrollment Form

Please fill out all required fields for the DoD Fresh Fruit and Vegetable Enrollment Form. This form is specially for sponsors who are interested in using their entitlement for the DoD Fresh Fruit and Vegetable Program for school year 2026-27.

Submission Deadlines for Participation:
  • February 27, 2026 (First round to start ordering July 1, 2026)
  • March 2, 2026 through December 10, 2026 (Requests will be processed ever few months)
  • January 15, 2027 (Last opportunity to join the program for current school year)

Please note: This form is to be completed by the Designated Official only (Program or Ordering Contact per CNPweb).
