We require two (2) personal references for each program. These personal references must not be relatives or reside in your home.

Applicant Attestation – Skilled Companion Dog Program

I understand and acknowledge that dogs placed through the Dogs Inc Skilled Companion Dog program are not service animals and do not have public access rights under the Americans with Disabilities Act (ADA). I further understand that these dogs are not covered under Fair Housing protections and are subject to standard pet policies, fees, breed or size restrictions, and housing approval requirements set by landlords or housing providers.

I agree that it is my responsibility to ensure I have appropriate and ongoing permission to have a pet in my home and to comply with all applicable housing, travel, and public access policies. I understand that misrepresenting a Skilled Companion Dog as a service animal or assistance animal may result in removal from the program.
