Welcome to the Wisconsin Maternal Mortality Review (MMR) Program Partnership and Implementation Survey. The MMRT develops recommendations based on in-depth reviews of pregnancy-related deaths in Wisconsin. These recommendations aim to prevent future maternal deaths and improve the health and safety of birthing people and families across the state. We’re inviting organizations, providers, systems, and community partners to share how your work may align with or support these recommendations.

Your responses will help us:
Identify where and how recommendations are being implemented across Wisconsin
Better understand who is doing this work and which populations are being reached
Strengthen connections among partners
Tailor future support resources and communications to community needs.

The survey takes approximately 10 minutes to complete. Please note that the survey may time out if left inactive for an extended period, so try to complete it in one sitting.
