ICHD CCBHC Needs Assessment: Community and Stakeholder Survey

Introduction

Thank you for being a valued Iroquois Center for Human Development region community member!

We ask that you take a few minutes to complete a community needs assessment survey administered by The Learning Tree Institute at Greenbush. This survey will be used to tailor services to the needs of our community. Participation is voluntary and anonymous. You will be able to skip questions you do not want to answer.
