Before diving into this comprehensive application, please ensure you have the following information at hand:
- Contact information for your Primary Care Physician
- Contact information for your Mental Health Provider
- Copy of your DD-214 - if applicable
- Copy of a DD-2870 - if applicable (click HERE for a blank form to complete)
- *Copy of the DD-1300 (see instructions below for obtaining)
- You will be asked to complete a PCL-C (PTSD Checklist - civilian version) if you are applying for PTSD mitigation

*To get a DD-1300, you will need to contact the National Archives or the relevant military branch and provide the necessary information along with proof of death and your relationship to the service member.
Requesting a copy
- From the National Archives:
Visit the National Archives website and use the online eVetreX system to submit a request, or mail a request using the SF-180 form.
- From the specific military branch:
If you know the service member's branch, you can contact the branch directly, such as the Navy's LTAP program.
