This free service is exclusively available to Anthem members in Alpine, Amador, Calaveras, El Dorado, and Sacramento Counties.
Eligible members get all of these additional services for free:
- Enrolling in Medi-Cal
- Understanding prescription drugs
- Support with finding and keeping housing
- Finding doctors and making appointments
- Connection to local resources and services
- Help with accessing mental health or substance use care
- Getting follow-up services after leaving the hospital
- Setting up transportation to your doctor visits
Eligibility
- Medi-Cal Insurance
- Enrolled in or eligible to enroll in the Anthem health plan
- Eligible Conditions or Situations:
- Transitioning from incarceration back to the community
- Enrollment in California Children’s Services (CCS)
- Frequent emergency room or hospital visits
- Involvement in the child welfare system
- Homelessness
- Mental health condition
- Substance use disorder

DocumentationTo help expedite the referral process, we encourage referring parties to submit any relevant supporting documentation alongside the referral form. We will review community referrals and use available data to confirm the individual’s eligibility for ECM. If additional information is needed to complete the review and approval, we will reach out for additional information and/or documentation.
