Before diving into this comprehensive application, please ensure you have the following information at hand:
- Contact information for your Primary Care Physician
- Contact information for your Mental Health Provider (not required for light mobility)
- A copy of your DD-214
- If you receive services through the VA - Completed copy of a DD-2870 (click HERE for a blank form to complete)
- You will be asked to complete a PCL-M (PTSD Checklist - military version) if you are applying for PTSD mitigation
