Provincial Model of Care for Sickle Cell Disease - Feedback Survey

Introduction

Ontario Health invites your feedback on a draft Provincial Model of Care for sickle cell disease. This invitation is being shared with key partners across the province, including individuals living with sickle cell disease, caregivers, clinicians, administrators, community members, and advocacy organizations. We welcome you to share this invitation widely within your networks to ensure diverse perspectives are included in shaping this important model. Over the past year, Ontario Health’s Quality Division has made continued efforts to expand and support implementation of the Sickle Cell Disease quality standard across regions in Ontario through a coordinated approach. 

The quality standard is one of Ontario Health’s deliverables on the Black Health Plan and furthers the goals of Ontario Health’s Equity, Inclusion, Diversity and Anti-Racism Framework.

This survey will be open from October 21, 2025 to November 5, 2025. The closing time is 11:59 (ET) on November 5, 2025.

This draft will be revised with your input and with ongoing feedback from the Ontario Health Sickle Cell Disease Implementation Advisory Group. The created model will be shared through the Sickle Cell Disease Community of Practice in early 2026.

Thank you for your work in helping to develop and establish a well-coordinated provincial model of care to facilitate the implementation of the Sickle Cell Disease quality standard and ensure high-quality care for people living with sickle cell disease in Ontario.
