Vermont: SUPRT for Recovery Services
Substance Abuse and Mental Health Services Administration (SAMHSA) has replaced the Government Performance and Results Act (GPRA) survey tool with the Substance Abuse and Mental Health Services Administration (SAMHSA) Unified Performance Reporting Tool (SUPRT).
This goes into effect October 1, 2025.
This survey is has two parts: 1) SUPRT-A; and 2) SUPRT-C.
SUPRT-A is the administrative section that is completed by the recovery coach conducting the interview with the client. This section can be completed without the client by consulting any records pertaining to said client.
SUPRT-C is the client section and it is to be completed by the client, with assistance from the recovery coach if needed.
