2026 Pennsylvania LGBTQIA+ Health Survey

Survey Introduction:
This survey is designed to better understand LGBTQIA+ Pennsylvanians' health. The goal is to gather information to better understand the wellbeing and health care experiences of you and your community. We will summarize the results so your feedback can be used to start conversations, inspire action, and bring attention to community needs. Reports from other years can be found here.

You can take this survey if you are LGBTQIA+ (lesbian, gay, bisexual, transgender, queer, intersex, asexual, etc.), live in Pennsylvania, and are 13 years or older.
This survey is collecting information to support improvements in care and services. Some of the questions may be sensitive (e.g., mental health, sexual health, violence, discrimination). You may stop the survey at any time. You may skip any question except for the three questions on this page, which determine survey eligibility.

Privacy:
Your responses to this survey are anonymous. We will not ask you for your name, address, or phone number. All responses will be kept confidential. We do not record your computer user IDs, IP addresses, or other tracking information. You will not be identified in any way. Saved data will be stored securely.

This survey is sponsored by the Pennsylvania Department of Health. PHMC, a non-profit organization, is conducting the survey and summarizing all findings.

Sensitivity:
There are some sensitive questions in this survey (including topics such as self harm and substance use). All questions are optional and can be skipped. If you ever feel uncomfortable about a question, just skip it.

Time:
Please complete the entire survey in one session, so none of your experiences, opinions, and ideas are lost. This survey should take less than 20 minutes to complete.
