2025 ODBH Problem Gambling Services Survey

Survey Introduction/Advance Communication

You are invited to participate in a survey conducted by Ohio Department of Behavioral Health (formally known as Ohio Mental Health and Addiction Services) and the Pacific Institute for Research and Evaluation (PIRE) that aims to collect data about the current problem gambling (PG) support service systems and programs that are offered at your agency.

We are asking individual service providers (e.g. counselor, therapist, nurse, peer recovery supporter, and others) - working in the areas of behavioral health prevention, early intervention, and treatment to complete a short series of questions that focus on the topics/areas of:
  • Programs and Services
  • Community Support
  • Marketing
  • Staffing
  • Funding
  • Training/Workforce Development
The survey should take approximately 10 minutes to complete. Please answer each question based on your experience and perspective.

We appreciate your valuable survey input that will help your state leadership work towards improving problem gambling services across Ohio. Your responses will help ensure that services meet the needs of people and communities impacted by gambling-related harm.

Please remember that your participation in this survey is voluntary, and you may choose to skip any questions or withdraw at any time without penalty.

Confidentiality
Complete discretion will be exercised. All responses will be kept confidential, your identity will not be linked to your responses, and any information collected will be stored securely.
