NAM CWB Healthy Provider, Healthy Patient Voices Survey

As part of our "Healthy Providers, Healthy Patients" campaign, the NAM Clinician Well-Being Collaborative is seeking to spotlight real-life stories on:

Positive relationships that you encountered within the health care system, as the patient or provider
Times when you experienced shared decision-making in health care
Examples of clinicians feeling they had agendcy to act in support of quality and safety
Moments when a clinician intervened over the course of care -- catching or reporting an error, sharing needed information for confident choices and active participation, or helping navigate towards better outcomes
And more!

Please use this form to share your vision for a world where patients and providers can be powerful partners in health care, in connection to Patient Safety Week in March 2026. The NAM will be featuring selected stories on our Collaborative webpage and social content.

1. We invite you to share a story about a meaningful patient-provider interaction you've had, whether you were the provider or the patient (approx. 50 words).

These stories can be positive or unsatisfactory. If the former, consider drawing inspiration from themes featured by the Collaborative, including teamwork, shared decision-making, active listening, and informed participation in care. If the latter, you can share what you would have wanted to see -- tell us about the opportunities within your experience where the patient-provider relationship could have been strengthened and how this support could have made a difference towards a better, safer outcome. Feel free to depict this as a hope or a dream! Note: responses may be edited for length / style guidelines / clarity.
